Wastewater Discharge Permit Application Forms

The Warwick Sewer Authority’s (WSA) Industrial Pretreatment Program (IPP) permits an assortment of industrial and commercial facilities located within the City of Warwick. You don’t have to be an industry to have materials of concern in your wastestream. Our IPP must also consider chemicals stored and/or used in non-wastewater related processes at your facility; these chemicals have “potential” to be discharged accidentally to our collection system and must be stored and disposed of properly.

The WSA’s IPP has developed several different Wastewater Discharge Permit Applications that allow us to properly characterize the wastestreams of various, diverse, industrial and commercial facilities. Some applications are very specific in targeting key items of concern associated with a particular type of business or businesses. Other applications are more generic in questioning and thus cover an assortment of businesses.

Please review the application descriptions and examples of businesses provided in the table below to determine the Wastewater Discharge Permit Application which is best suited for your business/operations. Please note that the examples provided in the table are not all inclusive of the types of businesses permitted by the WSA. Examples are meant merely to provide a guide in determining the most appropriate application for your use. Food Service Establishments

This application should be used by those businesses engaged in food preparation and sale of food products. Contact IPP personnel for the proper application fee that applies to your business.

Restaurants, Delicatessens, Sandwich Shops, Convenience Stores, Supermarkets, Bakeries, Drive-thru/Fast-food, Pizza shops, Coffee shops, Wholesale/Retail Food Prep Facilities

Dental Facilities

This application should be used by all dental offices regardless of specialty.

All dental offices regardless of specialty must use this form.

Hospitality Industry

This application should be used by those facilities providing lodging and/or event services.

Hotels, Motels, Banquet Facilities or other facilities that have their own private laundering services and/or food service on-site.

Brewery, Winery & Distillery Industries

This application should be used by those facilities in the brewery, winery, or distillery business.

Breweries, Wineries, and Distillers.

Professional Health Care Facilities

This application should be used by those businesses providing health care services for residents at the facility.

Nursing Homes, Assisted Living, Rehabilitation Facilities

Other Industrial & Commercial Facilities

This application should be used by those facilities that have non-toxic wastestreams and/or may store pollutants of concern on-site at their facility

Automotive Repair & Maintenance, Marine Repair & Maintenance, Carwashes, Print Shops, Job Shops, Public Laundries & Dry Cleaners, Fabrication Shops, Landscape Business and Marijuana Cultivators

Toxic Organic Management Plan

This application should be used by any facility which meets one of more of the following criteria:

 

Business is subject to Categorical Pretreatment Standards under 40 CFR

 

Business discharges an average of >25,000 gallons per day of process wastewater or contributes a process wastestream which makes up to five percent (5%) or more of the average dry weather hydraulic or organic capacity of the Treatment Facility

 

Business has a reasonable potential, in the opinion of the Authority, to adversely affect the operation of the Treatment Facility and/or to violate any Pretreatment standard(s) or requirement(s).

Electroplating, Copper Forming & Etching Facilities, Refiners, Hospitals, High-flow Processing Plants, Facilities using Heavy Metals and/or Cyanide in their operations
